Form maker lite - name

witam, na stronie:

zastosowałem form maker lite, wyświetlany z menu "kontakt" za pomocą rokbox 2 jako popup.
formularz wyświetla się, ale gdy wypełniam pola i klikam na "submit" pokazuje się komunikat:
""name" field is required", a ja przecież wypełniłem wszystkie pola.
Niestety nie mam pojęcia jak rozwiązać problem. Czy mógłby mi ktoś pomóc? Z góry dziękuję za odpowiedzi.

The popup that opens in your page is not creating an iframe then loading form into that iframe. It just copies the form and thus you have 2 forms with the same id-s in your page which is impossible as the id-s must be unique in the scope of that page, that's why it's not working. We remove second form.

ktoś miał już taki problem, tylko sam nie wiem jak wykożystać rozwiązanie. jak to zrobić??
